SPK Terminal Illness Withdrawal
Members are eligible to withdraw from their SPK Member Account if they are verified as having a life-limiting illness by a Medical Board Report.
Upon submitting a withdrawal application, the Member with a life-limiting illness must be informed of the following:
The Member must obtain verification from the Medical Board confirming that he or she is diagnosed with a terminal illness.
The withdrawal payment to the Member will be made in a lump sum.

Ensure the documents required are complete before proceed with submission.
A complete withdrawal application form.
Copy of Member’s Identity Card.
Medical Board Report.
Letter of Undertaking by Guardian of Medically Unfit or Unsound Mind Member of The Fund (if required).
Copy of bank account details/statement or bank book.
